The prescribed supervisor’s checklist confirms the details of a designated building product being installed on a designated building.

The checklist must reflect the information in the approved documentation and note:

  • name of the designated building product
  • location of the product on the building
  • installation and fixing method used.

The checklist must be completed by the registered building work supervisor responsible for the installation of a designated building product.

The supervisor must sign the completed checklist and submit it to the council on the day of installation commencement (within one business day after notice of commencement is given to council).

A checklist must be submitted for each designated building product. Separate  checklists are required when multiple products are used on one building. When the same product is used in multiple locations on the same building, a checklist is only required for the first installation but multiple checklists can be submitted.

The prescribed supervisor's checklist is an approved form for the purposes of Regulation 93(4) of the Planning, Development and Infrastructure (General) Regulations 2017.
